THF AF3+2 set procs on Triple Attack, so that cannot be set bonus. You probably had just used Bully and SA, but hadn't attacked yet. SA applies to counters, but does not where off.

So anyone tried /mnk. Pop NM, turn Counterstance, Bully NM, target some mob around you (still facing NM), turn SA, enjoy 1000+ counters?

It was because of Bully+Sneak Attack. I thought about the set bonus only proccing on Triple Attack shortly after I posted this but it didn't seem important enough to come back and change it. Did not know about Sneak Attack not wearing on counters though so that's good to know.

Not necessarily Bully-SA, but also he could've been TA-ing someone in front of him with GH atma. Happened to me few times, fun times.
