I might be missing a few things.

Patricians Cuffs instead of Masters when you can't use them etc. Could use EVA+ Weapons and crap but I didn't add those.

Though I guess, you could throw in Melody Earrings +1, if you have songs and crap on

Could use a few fixes, Chrono. For instance:

Antares Harness would give the same amount of EVA, but also technically a Parry boost so it would be better for an Evasion set, technically.
Raven Hose are the best possible EVA pants for THF, and the Enmity reduction is negligible since most of the stuff you'd need an EVA set for don't require you to fight for hate.
There are a few different daggers strewn about, such as Ermine's Tail and Zareehkl Jambiya.
Other than that, lol...Wivre Mask +1 offers a slight boost in EVA over O Hat, but it's 900k on my server so it's up to you.

I may be missing one or two items but I think I got the majority.

I could get an Emp Pin or Wivre Mask +1 or Adder Jambiya +1 or possible augmented alternative. Been looking for a Triton Earring for awhile. I'll be rebuying my Antares Harness any time now. I could go for another Wivre Ring +1 though there might be other newer (augmentable?) alternatives, I'm not sure. I have Master Caster's Mitts, but I just leave em in storage.

On Eva Skill vs Eva, I'm in the camp that things that Eva Skill = 0.9 Eva past 200 skill, truncated. That makes it worse than normal points of evasion. But nobody knows how this stuff works for sure though. But that's why I choose Dance Shoes +1 and Musical/Elusive over alternative.

I use Optical Hat because it doubles as my ranged accuracy piece and also has dmg stats on it, which is at a point becomes better than an extra point of evasion, even for your max evasion set (unless you want an evasion set for when you're never engaged to something, ie kiting & pulling).

There are lots of hybrid evasion sets you can use too. I use lighter ones sometimes, and will sometimes throw in subtle blow, or other stats. Here are two more set types that I commonly use:

I'd use an eva+5 earring like ethereal over evasion earring. There's been nothing ever tested that shows that eva skill is actually more effective than evasion. All these tests have come so close that most just assume they do the same.

Edit: as for things like wivre mask, I'd still use ohat over it. If your acc is uncapped, you gain 5% hitrate, which means you kill more than 5% faster, less time to do something that'll end up screwing you up

I know that when I have used a serious evasion setup in the past on Darkmoose, I used the following:

Optical Hat
Evasion Torque
2x Bat Earrings (+ Blinding Potions)
Scorpion Harness (+1)
Grand Temple Knight Bangles
Boxer's Mantle
Scouter's Rope
Raven Hose
Powder Boots (See below)

Food - Jack O Lantern

I am missing some key pieces, like Ungur Boomerang and Emissary Boots. I have an Evasion Earring and Ethereal Earring (personally, I believe that 3 Evasion Skill is better than 5 Evasion from Elusive Earring), but it depends on what I'm doing. If I have a case where I'd be fighting, like Charybdis, I'd rather not be Blinded.

With this evasion setup and Blinding Potions, I can pop Jailer of Prudence and successfully get far enough away from the spawn to deagro it. That's the reason for the Powder Boots, by the way.

Perfect Dodge, pop JoP, Flee, run like hell, recast Utsusemi, run some more, pop Powder Boots, run like hell, recast Utsusemi, run some more.

I think that the last time I did this I had 100% HP when JoP deagro'd and I still don't have capped THF Evasion (Think I'm around 265 + 2 merits). It missed a very significant number of attacks, and between that, Perfect Dodge, 2 Flees, and Utsusemi, I came out with nary a scratch.

Used to Thf tank Za'Dha Adamantking in a low man group using this set:

Was able to Ni to Ni full time only having to use ichi a handful of times throughout the fight.

There are a few things that can be improved with the addition of new daggers and depending on your pt set up or if you are solo.

Earrings: http://www.ffxiah.com/item.php?id=14726 x2 if you have a brd. Would never use bat earrings personally because if you are blind you cant land bolts if needed and DE damage will be severly decreased which will makes it harder to tank if thats what you are doing.
Legs: Can use http://www.ffxiah.com/item.php?id=15579 if you are solo but if you are tanking the -enm is not recommended.

The Bat Earrings work on the "Lost Thief"
JoP strategy because you don't actually have to fight it, just pop it, run away, and survive long enough that the one that you had following you despawns so the rest of the group only has to fight one copy.

In that case, one Blinding Potion will last just long enough to get the job done.

Duoing Charybdis with a BRD, you can actually use Blinding Potions also, or just let the Blind from Ink Jet stay on you. I Barded a NIN solo tanking Charybdis once who had a solid evasion setup with Blinding Potions and Bat Earrings, and his accuracy was still very solid.

If you are evasion kiting something, Bat Earrings are good, but if you need to have all the accuracy you can get, I'd go with something else.

As is the case with everything in FFXI, there are small tweaks that you need to make for each individual situation. No one setup is perfect for everything.

Well evasion earring is about 6 evasion.
