It depends... Most you meet will be salaried and will be employed by a public or private institution. Ones that work off of patient intake are usually running their own practice...

I used to work at a chiropractic office doing insurance. Basically, if a patient had insurance coverage, we (the office) got paid whatever the insurance company deemed appropriate. They have set values for each procedure (its called a fee schedule), so no matter how much the doctor charges, the insurance sends in that amount to cover the visit.
If a patient doesn't have insurance, or their insurance doesn't cover chiropractic, then the doctor would charge however much they wanted. Most offices have a set "cash" value for the visit, which can and usually is, more than what the insurance would send in.
Medicare, Medicaid, Medical all have the lowest fee schedule, meaning that a $100 visit, would net the office about $20 in coverage. However, most Medicare patients have supplemental insurance as well. This means once Medicare has paid, you can submit a secondary claim to get additional money to cover the rest of the visit.

However, in regular medical situations, this is not always the case. If you were to go to the emergency room, they would bill you later for the services. So there is a large pool of patients "in collections" because they haven't paid their bill.

yeah, on the brokerage level... lol sometimes we get some pretty wild stuff that comes in though lol... I can't handle all the numbers all the time with you accountant peoples!

When I took photography in high school we had to research a photographer and one I picked was first known for self pictures in which she would disguise her self as someone else... I thought that was neat so started looking into her stuff and found out she did a clown series. One of the pictures I highlighted on the school computers and it inverted the colours... that clown still visits me in my dreams >.>
She also did a serise with maniquins being arranged in dirty ways but that was just fun from peoples reaction glancing over at my screen.
