It is, according to people who received letters, you are banned from all future online services by them, and they also have a policy of wanting to make a lasting effect on people who violate their ToS.
Example: Don't ban RMT fishermen selling lu shang's until they get the rod, then kick them off immediately after, etc, etc.

Techiniqually yes. However you do that alot and it's gonna look pretty damn bad. Bad publicity=less moneys. Also theres some work being done on players legal rights so you never know.

Gettin a new credit card number isnt really that extreme. Hell I'd argue that denying someone service of FFXIV because of actions some people may or may not have done in FFXI much more extreme.

They only ban by credit card if the banning is RMT related. So they wouldn't check for the same CC number. Same for address. How they would enforce this, I don't know. But it wouldn't be from CC or Address or anything personal.
