Some in first world counties don't even have homes at all tbh. People getting state benefits have free dental health care in the UK, they still don't go very often until they have to...heck some don't even go then. They don't joke about the brits teeth for no reason, lol.
Sadly here they don't put too much importance on dental care, most plans don't include it.
