If I am the one who wrote the questions for an online survey for my own paper is it okay to put myself as the author in my own works cited?
I would suggest using the online survey that you got it from. If there's no author, don't worry about putting one in there.
Other than that, you don't need to cite yourself. That'd be like dividing by zero. Unless of course your professor is expecting you to be a smartass (like I was in my last paper). In that case, this is what I did: Me, . "Me." Me. None.Nope (Now): What?. Print.

Just for the works cited I need to put an author so I figured I would just put my name there. always forget how to upload pictures to guildwork :/
